Wish Qualifications

Must be at least 18 years of age.

Must have a terminal illness with a prognosis of 6 months or less as certified by a medical professional.

Must be medically, physically, and cognitively aware and able to participate in the wish in a meaningful way.

Before we can process any Wish Applications, we must receive the following completed forms. For your convenience, we offer secure digital forms that can be easily e-signed from any location—right from your mobile device or computer. Once you have clicked "Apply for a Wish" and completed the initial Wish Application one of our representatives will reach out to you via E-mail with next steps.

Physician Form Requirement
As apart of your initial wish application, please discuss your wish with your Physician and please direct your Physician to complete the required "Certification of Illness and Medical Waiver". This form will be required to proceed with your wish application consideration.
